I am trying to get the literal string '&' to print out as just that
and not the & symbol. I have tried using both the <literal> and
<markup> elements, but they both result in the & symbol as output. Does
anyone know what I am missing?

For that matter,

<![CDATA[&]]>

should work as well. The other is probably simpler, though.
